DSO Registration Form

To start the application process your area DSO has to see if you are eligible. You will need to provide us with documents to prove that you have a developmental disability, live in Ontario, and are 16 years old.

You may call your area DSO or register with us online to begin the application process. When you register online you will be able to securely upload your eligibility documents. You will also receive notifications when your information is being reviewed by your area DSO and receive information on services and supports that are of interest to you.

Account Information

Your password must be at least 8 characters long and contain at least one CAPITAL letter, one number and one of the following special characters: !@#$%&^*?

Please enter a valid and functioning email address. This is needed so you can receive your account activation email and other important email notices from the DSO Client Portal. In the event you do not receive your account activation email, please check your spam folder.
















* Please note if you are not a caregiver, you will have to complete a third-party consent, please contact your area DSO to complete the same.





Applicant Information

In case you are moving soon or need to change your selected DSO, please call your area DSO.

Choose the DSO to contact by manually selecting one of the counties in which DSO operates.
The DSO to contact is selected automatically based on your postal code. You may if necessary (e.g. if you are moving in the near future).





Has the applicant had a psychological assessment? To prove that you have a developmental disability, you will have to give us a copy of your psychological assessment (signed on all pages). A psychologist or psychological associate would have met with you, and you would have done some different activities to see what you can do, how you behave, and how you think. Do you have a copy of this?



Caregiver/Professional/Referral Source Information
