STEP #16
Let’s take a look at this checklist to see if we’ve covered everything you need to know to get access to services…
- If you have registered an online account and submitted all your personal documents electronically, you will get a confirmation letter from your area DSO telling you if you are eligible to apply for services.
- If you did not submit your documents electronically through your online account, then follow these steps:
- Get all your personal documents ready that prove that you are over 16 years old, live in Ontario and have a developmental disability.
- Make copies of all these documents and send them to your area DSO by fax or mail.
- Shortly after you give your area DSO your information, you will get a confirmation letter from them telling if you are eligible to apply for services.
- You will get a call from someone from your area DSO. They will ask you when you are available to meet.
- You will have 2 meetings with someone from your area DSO, each meeting will take about 3 hours to finish. We need to have these meetings so that we get to know your goals and needs.
- Once we know your needs we will connect you to services that are available in your area.
- It may take some time, so please call us every year, in the month that you celebrate your birthday or anytime, let us know if anything has changed in your life. Some examples include, a change of address, a change in your health or a caregiver’s health.
